1. An applicant must appear in person and submit a completed passport application (Form DSP-11).
The application must be typed or printed clearly in black ink. DO NOT SIGN the application until you are instructed to do so by the Passport Acceptance Agent.

If the application is for a minor child 15 years and under: The minor and the person applying for the passport on behalf of the minor must appear in person and submit a completed passport application (Form DSP-11). Effective July 2, 2001 Public Law 106-113 requires both parents or legal guardian(s) signatures on a passport application for a child under the age of 15, or notarized consent documentation of the absent second parent. Parents, legal guardian(s), whether a U.S. citizen or not, must personally appear and execute an application for a child who is under 15 years of age, or the applying parent must show proof of sole custody (i.e. - legal + physical = certified copy of court order is required) the agreement of the second parent, or death certificate. 

Applications for children outside the U.S. must be made at the nearest U.S. consulate or embassy in the country where the child is located. The person applying on behalf of the minor must be a parent or legal guardian. If the person applying for the minor is acting on behalf of a parent, they must provide a notarized statement from the parent giving them permission to execute the passport application for the child. The application must be typed or printed clearly in black ink. DO NOT SIGN the application until you are instructed to do so by the Passport Acceptance Agent.
 
2. Submit Proof of U.S. Citizenship such as
U.S. Passport - a previously issued U.S. Passport, if it is less than 15 years old from date of issuance. To inquire as to whether or not you qualify for a mail-in application (see RENEWAL OF PASSPORT BY MAIL).
OR
Birth Certificate
- a certified copy of a birth certificate issued under the authority of the appropriate registrar of births shall be presented. To be acceptable, the certificate shall bear the written signature of the registrar or other custodian of birth records and the impressed, embossed, raised or multi-colored seal of the issuing office. It must show the given name and surname, the date and place of birth, and that the original birth record was filed within 1 year of the date of birth. (NOTE: California Birth Abstracts are not acceptable.) HOSPITAL CERTIFICATES AND BAPTISMAL CERTIFICATES ARE NOT ACCEPTABLE. Naturalization Certificates
- a naturalized citizen must submit their naturalization certificate. 
 
3. Submit Proof of Identity  
An applicant must show acceptable identification such as a valid driver's license (no temporary, expired or learners permits), military I.D., Department of Motor Vehicles I.D., employment badge if it has a photograph, signature, issue and expiration date. A parent with acceptable I.D. may identify minors.  
 
4. Submit Two Recent Color Photographs  
Applicants may purchase their photos from the Clerk-Recorder for $14.00.

If submitting other passport photos, they must be 2” x 2”, with a white or off-white background, and the head measured from the bottom of the chin to the top of the head (including hair) shall not be less than 1” nor more than 1 3/8”.

Only 2 photographs need to be submitted if an applicant is applying for both a passport and passport card at the same time.
