Passport acceptance facilities cannot accept passport renewal applications (Form DSP-82). Click here to get more information on Passport Renewal by Mail.
 
To renew a passport by mail, the applicant must
1.
Complete and sign an "Application for Passport by Mail" (Form DSP-82).
2.
Submit the most recent U.S. passport issued to him or her within the past 15 years; and
3.
Have been at least age 16 when the previous passport was issued; and
4.   
Use the same name as in the previous passport or be able to present a certified marriage certificate or court order changing the name. If the name has been changed other than by marriage or court order, the customer must use Form DSP-11 to apply. When using a DSP-11 form, you must appear in person at an acceptance facility.
5.  
Submit 2 identical photographs of applicant, in color. Photographs must be 2" X 2" in size. Passport photos may be obtained from the County Clerk-Recorder's office for $14.00.
6.  
Mail the above items, along with a check, cashiers check or money order in the amount of $110.00 payable to U.S. Department of State, to the address listed on the Form DSP-82.
